Thank you for your interest in the Utah health survey!
The past few years have shown Utahns how important it is to support public health. To improve the health of Utahns, the Utah Department of Health and Human Services would like to better understand factors that affect adults’ beliefs and attitudes about physical health and tobacco use. To do this, the Utah Department of Health and Human Services is conducting a survey in partnership with RTI International, an independent nonprofit research organization.
You can help by completing this important survey. The survey is short and will take about 15 minutes. The adult (age 18 or older) in your home with the next birthday should complete the survey. Simply enter your 6-letter access code on the on the home page to get started.
Here are a few important details about the survey:
- Participation is voluntary and will help us understand adults' beliefs and attitudes about physical health and tobacco use in Utah.
- Completing the survey generally takes 15 minutes and you only need to complete the survey once.
- There are no physical risks involved in participating in this survey. You will be asked about illegal cannabis use behaviors and while answers will be kept confidential, disclosing cannabis use behaviors could pose legal or reputational risk.
- Respondents can skip questions they find uncomfortable or embarrassing to answer.
- The information you provide will be kept confidential to the extent allowable by law. RTI will have access to participants’ identifying information however no identifying information will be shared with the Utah DHHS or other organizations.
- There are no direct benefits to participating in the study but you will receive a monetary incentive after completing the survey.
