This survey focuses on health. We want to better understand factors that affect adults’ health status and their beliefs about health conditions.

The survey should take about 15 minutes to complete.

No. Taking this survey is completely voluntary. But it is important that we include your views and experiences. To ensure the accuracy of survey results, we need to complete surveys with as many eligible Utah residents as possible.

No, unfortunately we’re only surveying Utah residents. You can let us know that you’re not eligible for the survey, by doing one of the following:
  • Enter your survey access code on the survey website (uthealthsurvey.rti.org) and for the first question, select “There are no adults eligible for this survey in this household.”
  • Email UTHealthSurvey@rti.org and provide your survey access code or mailing address.
  • Call the survey manager at RTI International at 877-254-1840. If you leave a voicemail, please include your survey access code or your mailing address.
After you complete one of these steps, we will remove you from future mailings.

This is your opportunity to tell us your views and experiences related to health and tobacco use. If you complete your survey online, we will also send you a check or VISA gift card for $20 dollars, as a token of our appreciation for your participation.

We randomly selected your address from a list of all addresses in Utah.

The survey includes questions on health, use of tobacco products, and exposure to secondhand smoke. The survey also includes questions about illegal cannabis use behaviors and your views about public smoking policies and anti-tobacco campaigns you may have seen on television or online.

No. This survey is only being conducted among Utah residents.

RTI International is a not-for-profit research organization based in Research Triangle Park, North Carolina. RTI partnered with the Utah Department of Health and Human Services to create this survey.

Your Login ID can be found in any of the letters or postcards you received from the Utah health survey. If you have misplaced these letters and postcards, please email UTHealthSurvey@rti.org or call 877-254-1840.

Absolutely not. Your name and contact will be kept private and confidential.

Yes, but only your answers will be shared. They will not receive any of your personal information so no one in the government will know who gave the answers.

Yes. Information collected in the questionnaire is encrypted at all times.

Yes. The information that you provide will be de-identified, which means that it will not be connected to your name or other personal information.

Yes. All of the information you provide will be kept strictly confidential. We appreciate your willingness to provide answers and we take safeguarding this information very seriously.

No. Your address was randomly selected for this survey.

Yes. You were randomly selected to represent thousands of other Utah residents who were not selected to participate. We need to include adults with all kinds of experiences and views on smoking and health, including those that do not smoke.

Our survey protocols require that we select one adult from your household to interview. We ask this question to randomly select one person and ensure that the study represents all adults in Utah.

We are just looking to interview one adult in each household, in this case the one with the next birthday. This random selection helps to ensure that our participants reflect the population of the entire state of Utah, not just people who are easiest to contact or most willing to participate.

Unfortunately, only those who were invited to participate in this study may complete the survey. By limiting the people who may participate to those who were randomly selected, we ensure that the results are scientifically valid and accurate.

At the end of the survey, we will ask you for your name and address information so that we can send you your check or VISA gift card in the mail. This may take up to four to six weeks to arrive in your mailbox.
